This 6 month tracker is designed to be a place for you to be able to document the frequency of your (or your loved one’s) urinary tract infections (UTIs), identify behaviors that may put you at risk for UTIs, track UTI symptoms, and record your UTI treatment. With all of this information in one place, it will help you be the best advocate for yourself while you are at your doctors appointments. This will also hopefully serve as a tool to help minimize the overprescribing of antibiotics.
